Job Description
Lebanon Center, a skilled nursing and rehabilitation facility in Lebanon, NH, is seeking a compassionate and dedicated Certified Nursing Assistant (CNA) to join our Healthcare & Medical Services team.
In this resident-centered role, you will provide direct hands-on care and daily support to individuals in short‑term rehab, long‑term care, and memory care settings.
Working under the supervision of licensed nursing staff, you will assist residents with activities of daily living such as bathing, dressing, grooming, toileting, feeding, and mobility, while promoting comfort, safety, and dignity at all times.
You will take and record vital signs, observe and report changes in residents’ condition, assist with restorative and rehabilitative activities, and help maintain accurate documentation in accordance with care plans and regulatory standards.
The CNA will support residents with safe transfers and positioning, use proper infection control and standard precautions, and help maintain a clean, organized, and safe environment.
You will collaborate closely with nurses, therapists, and other team members to ensure high‑quality, personalized care tailored to each resident’s needs and goals.
As a key member of our collaborative care team, you will build trusting relationships with residents and their families, offering emotional support, respectful communication, and reassurance throughout the care journey.

Requirements
Active state CNA certification.
Prior long-term care or hospital experience preferred — new CNAs welcome.
BLS preferred, not always required.
Ability to stand for long shifts, lift up to 50 lbs, and assist with patient transfers.
