Job Description

Lebanon Center, a skilled nursing and rehabilitation facility in Lebanon, NH, is seeking a compassionate Licensed Nursing Assistant (LNA) to join our Healthcare & Medical Services team.

In this resident-centered role, you will provide direct, hands-on care and support to adults and older adults requiring short-term rehabilitation, long-term care, memory care, and specialty clinical services.

Working under the supervision of licensed nurses, you will assist residents with activities of daily living such as bathing, dressing, grooming, toileting, feeding, mobility, and safe transfers, ensuring comfort, dignity, and respect at all times.

You will take and record vital signs, measure height and weight, observe and report changes in residents’ condition, assist with restorative and rehabilitative activities, and support residents in following their care plans.

You will help maintain a clean, safe, and organized environment, follow infection control and safety protocols, and use proper body mechanics and equipment for lifting and transfers.

As a key member of a collaborative care team, you will communicate respectfully and effectively with residents, families, nurses, therapists, and other staff, helping residents maintain independence and quality of life.

You will document care accurately and promptly in accordance with facility and regulatory standards and participate in shift reports and interdisciplinary care discussions.
