Job Description
Lebanon Center, a skilled nursing and rehabilitation facility in Lebanon, NH, is seeking a compassionate and dedicated Certified Nursing Assistant to join our Healthcare & Medical Services team.
As a CNA, you will provide hands-on support with daily living activities, including bathing, grooming, toileting, feeding, mobility assistance, and positioning to promote comfort, dignity, and safety for residents in short-term rehab, long-term care, and memory care settings.
You will take and record vital signs, monitor and report changes in residents’ condition, assist with restorative and rehabilitative care plans, and support nurses and therapists with clinical tasks as appropriate.
Working in a resident-centered, collaborative environment, you will build trusting relationships with residents and families, offer emotional support, and help create a warm, respectful atmosphere that supports each resident’s quality of life and individual goals.
You will follow care plans, infection control procedures, and safety protocols, maintain accurate documentation, and participate in interdisciplinary team communication to support coordinated, high-quality care.

Requirements
Active state CNA certification.
Prior long-term care or hospital experience preferred — new CNAs welcome.
BLS preferred, not always required.
Ability to stand for long shifts, lift up to 50 lbs, and assist with patient transfers.
