Job Description
Signature HealthCARE is seeking a compassionate and detail-oriented Licensed Practical Nurse (LPN) to join our Healthcare & Medical Services team within our Healthcare & Medical Services department.
In this role, you will provide resident-centered, high-quality nursing care to patients in a long-term care and post-acute environment, supporting our mission to revolutionize healthcare through innovation, continuous improvement, and cultural enrichment.
The LPN will be responsible for delivering hands-on clinical care, including monitoring vital signs, administering medications and treatments as ordered, performing wound care, assisting with ADLs, and promptly reporting changes in condition to the supervising Registered Nurse and provider teams.
You will accurately document care and assessments in electronic medical records, ensuring compliance with regulatory standards, facility policies, and best practices in infection prevention and safety.
Working in close collaboration with RNs, physicians, therapists, CNAs, and other interdisciplinary team members, you will help develop, implement, and update individualized care plans that reflect each resident’s goals, preferences, and clinical needs.
You will educate residents and families on disease processes, medications, and care routines, promoting dignity, independence, and quality of life.
This position requires strong clinical judgment, excellent communication skills, and the ability to prioritize care in a fast-paced, field/on-site environment.

Requirements
Active LPN/LVN license in good standing.
6+ months of clinical experience preferred.
BLS required. IV certification a plus.
Ability to stand for long shifts, lift up to 50 lbs, and assist with patient transfers.
