Job Description
Signature HealthCARE is seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our Healthcare & Medical Services team. As an LPN with Signature HealthCARE, you will play a key role in delivering high-quality, resident-centered care in a collaborative, interdisciplinary environment. You will provide direct bedside nursing care to residents and patients, supporting their physical, emotional, and psychosocial well-being while upholding our mission to revolutionize healthcare through innovation, continuous improvement, and cultural enrichment. In this role, you will perform ongoing patient assessments, obtain and document vital signs, administer medications and treatments as ordered, and assist with wound care, catheter care, and other clinical procedures within your state-approved scope of practice. You will contribute to the development and implementation of individualized care plans, monitor residents for changes in condition, promptly report findings to the supervising RN or provider, and participate in care conferences to ensure that each resident’s needs and preferences are addressed. Accurate and timely documentation in the Electronic Health Record (EHR) is essential, as is maintaining strict adherence to infection control practices, safety protocols, and HIPAA regulations. The Licensed Practical Nurse will collaborate closely with RNs, CNAs, therapists, physicians, and other members of the care team to coordinate services, promote early intervention, and support positive clinical outcomes. You will provide clear communication and education to residents and their families regarding medications, treatments, disease processes, and self-care strategies, helping them make informed decisions about their care. You will also assist with admissions, transfers, and discharges, ensuring a smooth and respectful transition for each resident. Requirements
Active LPN/LVN license in good standing.
6+ months of clinical experience preferred.
BLS required. IV certification a plus.
Ability to stand for long shifts, lift up to 50 lbs, and assist with patient transfers.
