Job Description
As an LPN with Stellar Senior Living, you will play a key role in delivering high-quality, person-centered care to residents in our senior living community. Working within our Healthcare & Medical Services department, you will provide direct nursing care, administer medications, and support residents with their daily health needs in an environment that values compassion, dignity, and independence. In this role, you will complete nursing assessments, monitor changes in resident condition, and promptly communicate updates to the supervising RN, provider, and families as appropriate. You will administer medications and treatments according to physician orders and community policies, maintain accurate and timely documentation in electronic health records, and follow all regulatory, safety, and infection-control standards. You will collaborate closely with caregivers, med techs, and other team members to coordinate care plans, participate in care conferences, and help ensure that each resident’s physical, emotional, and psychosocial needs are met. As a clinical resource on the floor, you will provide guidance and oversight to unlicensed staff, support training and mentoring for new team members, and help maintain a positive, respectful, and supportive work environment. The LPN will educate residents and families about health conditions, medications, and care plans, promoting independence and informed decision-making whenever possible. You will respond promptly to resident concerns, changes in status, and emergencies, using sound clinical judgment and following established protocols. You will support the community’s efforts to reduce hospital readmissions, manage chronic conditions effectively, and maintain residents at their highest practicable level of function. Requirements
Active LPN/LVN license in good standing.
6+ months of clinical experience preferred.
BLS required. IV certification a plus.
Ability to stand for long shifts, lift up to 50 lbs, and assist with patient transfers.
