Job Description
Horizons Care Center seeks a compassionate Registered Nurse to provide high-quality, resident-centered care in our skilled nursing and rehabilitation facility.
The RN will conduct assessments, develop and implement individualized care plans, administer medications and treatments, document accurately in the EHR, and collaborate with an interdisciplinary team to support both short-term rehab and long-term care residents.
This role is ideal for nurses committed to dignity, safety, teamwork, and open communication who want to make a meaningful impact on residents’ quality of life while growing professionally in a supportive environment.

Requirements
Active RN license in good standing.
1+ year of clinical experience preferred; new grads considered for residency tracks.
BLS required. ACLS preferred for hospital roles.
Ability to stand for long shifts, lift up to 50 lbs, and assist with patient transfers.
