Your role will be rooted in the WellStar Professional Practice Model, 5-Star Nursing, and evidence-based practice. Core Responsibilities and Essential Functions Exemplary Practice and Outcomes Utilize the Nursing Process (assessment, diagnosis, outcomes identification, planning, implementation, and evaluation) to deliver individualized clinical care. Provide relationship-based, patient-centered care that respects cultural diversity and adapts to population-specific characteristics. Engage patients and their significant others in care planning, ensuring their goals are the focus of the plan. Adhere to current clinical practice standards to ensure optimal patient outcomes. Teamwork and Collaboration Coordinate the delivery and documentation of safe, quality patient care through inclusive interdisciplinary communication. Demonstrate teamwork by fostering a respectful and inclusive clinical environment. Participate in performance improvement initiatives and support shared governance at all levels. Professional Development and Initiative Complete all required professional competency assessments and ongoing education. Serve as a preceptor or mentor for other nurses and staff, modeling professional nursing practice. Evidence-Based Practice and Research Promote evidence-based practice and participate in nursing research and performance improvement activities. Uphold all healthcare system policies and clinical competencies, ensuring compliance with legal and accreditation standards. Engage in data collection and consult experts to address clinical practice questions. Resources and Support Proactively plan for patient care across the continuum while providing direct patient care.
