Job Description
Centers Health Care is seeking a compassionate and detail-oriented Registered Nurse to join our Healthcare & Medical Services team. In this role, you will provide high-quality, patient-centered nursing care across a variety of clinical settings, collaborating closely with physicians, therapists, social workers, and other care team members to achieve the best possible outcomes for patients. You will be responsible for conducting comprehensive patient assessments, developing and implementing individualized care plans, administering medications and treatments according to physician orders and facility protocols, and continuously monitoring patient status to identify changes in condition. The Registered Nurse will document all aspects of care accurately and promptly in the electronic medical record, ensuring compliance with regulatory standards, facility policies, and best-practice guidelines. You will educate patients and their families on diagnoses, treatment plans, medications, and discharge instructions, promoting understanding, safety, and long-term wellness. Working within an inclusive and collaborative culture, you will actively participate in interdisciplinary team meetings, contribute to quality improvement initiatives, and support evidence-based practice to improve clinical outcomes and patient satisfaction. You will provide leadership and guidance to Licensed Practical Nurses, Certified Nursing Assistants, and other support staff, delegating tasks appropriately and serving as a clinical resource on the unit. The role also involves maintaining a safe environment by adhering to infection prevention protocols, medication safety standards, and emergency procedures, as well as promptly reporting and documenting adverse events or changes in patient status. You will engage in continuous learning and professional development, staying current with nursing best practices, new technologies, and regulatory changes that impact patient care. At Centers Health Care, you will be part of an organization that values innovation, continuous learning, and teamwork. Although we are known for our forward-thinking mindset and use of technology, your primary focus as a Registered Nurse will be providing hands-on clinical care and compassionate support to patients and families. You will have opportunities to enhance your skills through ongoing training, mentorship, and exposure to new tools and care models that support more efficient and effective nursing practice. This position is ideal for a nurse who thrives in a fast-paced environment, demonstrates strong clinical judgment, communicates clearly with patients and colleagues, and is committed to delivering safe, empathetic, and culturally sensitive care every day.
