Job Description

Highlights This Advanced Practice Provider will join our growing team of providers in the Community ProCare at Home Program practicing in Indianapolis and the surrounding counties. This position will provide in-home house call services to patients who have previously been hospitalized, recently discharged from a skilled nursing facility, or are homebound. Ideal candidates will have demonstrated expertise in the advanced practice of nursing with chronically ill patients and have an understanding of the specific needs of assessing patients as they navigate their health challenges. Ideal candidates will also have knowledge of local resources as well as the ability and desire to work independently and genuine interest in delivering care in the home setting. Prior house call or home care experience is strongly preferred.
