Job Description
Life Care Centers is seeking a compassionate, patient-focused RN Registered Nurse to join our Healthcare & Medical Services team. As an RN with Life Care Centers of America, you will play a key role in delivering high-quality nursing care to senior residents and rehabilitation patients in a supportive, family-focused environment. You will use your clinical expertise to assess patient needs, develop and implement individualized care plans, administer medications and treatments, and collaborate closely with an interdisciplinary care team to promote optimal health outcomes. In this role, you will conduct comprehensive patient assessments, monitor vital signs, manage complex clinical conditions, and respond promptly to changes in patient status. You will provide skilled nursing care such as wound management, IV therapy, catheter and tube care, pain management, and support for rehabilitation therapies as appropriate to the care setting. Accurate and timely documentation in electronic medical records is essential, as you will maintain detailed, compliant records of all assessments, interventions, and patient responses. You will partner with physicians, therapists, CNAs, social workers, and other healthcare professionals to coordinate care, support discharge planning, and ensure each resident’s plan of care reflects their physical, emotional, and psychosocial needs. A key part of your work will be educating patients and their families on diagnoses, medications, care routines, and health maintenance, always reflecting Life Care Centers’ commitment to compassionate, patient-first service. The RN Registered Nurse will promote and model quality, safety, and infection control practices, actively participating in fall prevention, skin integrity programs, medication safety checks, and adherence to regulatory standards. You will help mentor and support CNAs and other team members, contributing to a culture of respect, teamwork, and professional excellence. This role offers the opportunity to grow your clinical skills in a dynamic long-term care and rehabilitation environment, backed by an organization dedicated to professional development and high-quality outcomes. If you are a licensed RN who values teamwork, clinical integrity, and meaningful relationships with residents and families, Life Care Centers provides an opportunity to make a lasting impact while developing your nursing career in senior living and rehabilitation care.
