Job Description

Lebanon Center Rehabilitation & Healing is seeking a dedicated and detail-oriented Licensed Practical Nurse (LPN) MDS (Minimum Data Set) Coordinator to join our team.

As an LPN MDS Coordinator, you will be responsible for overseeing the assessment and analytical processes within our facility, ensuring both compliance with federal and state regulations and alignment with our center's high standards of care.
