The Opportunity The Clinical Director (NP) at Pine Park Health combines clinical excellence with inspiring leadership, balancing direct patient care with provider management responsibilities. Clinical Responsibilities Establish meaningful patient relationships through comprehensive Welcome Visits and develop personalized Care Plans covering goals, diagnoses, medications, and advanced care planning. Ensure timely completion of Annual Wellness Visits focused on preventive care and comprehensive documentation. Provide seamless clinical coverage during provider absences, maintaining continuity of care. Conduct patient assessments, medication management, and follow-up care focusing on geriatric-specific needs. Respond promptly to urgent medical and behavioral health concerns to prevent unnecessary hospitalizations. Conduct post-discharge visits for patients transitioning from acute or skilled nursing facilities. Adapt workflows to accommodate urgent care needs while maintaining quality preventive services. Leadership Responsibilities Lead and develop a team of 6-8 Nurse Practitioners and Physician Assistants through regular performance reviews, mentorship, and professional development Ensure providers meet key metrics, including patient visit targets, documentation timeliness, and care gap closures. Drive community growth by attending pipeline meetings, leading implementation kickoffs, and building relationships with senior living facility staff.
