Job Description

Heritage Park Care Center is seeking a compassionate Licensed Practical Nurse to join our Healthcare & Medical Services team.

In this role, you will provide direct patient care, administer medications, monitor vital signs, and support residents with daily living activities in accordance with care plans and regulatory standards.

You’ll collaborate with RNs, physicians, and interdisciplinary staff to ensure quality, person-centered care.

Ideal candidates hold a current LPN license, strong clinical skills, and excellent communication.
