Job Description
AlignMed Partners is seeking a skilled and patient-centered Nurse Practitioner to join our Healthcare & Medical Services team in support of high-quality, innovative clinical care.
In this role, you will provide compassionate, evidence-based care to a diverse adult patient population, performing comprehensive assessments, formulating differential diagnoses, and managing both acute and chronic conditions in collaboration with physicians and the broader care team.
You will conduct patient histories and physical examinations, order and interpret diagnostic tests, prescribe medications within your scope of practice, and develop individualized treatment and care management plans that emphasize education, prevention, and long-term wellness.
Working within AlignMed Partners’ culture of collaboration and innovation, you will partner closely with physicians, nurses, allied health professionals, and practice leadership to streamline care delivery, close care gaps, and enhance quality and patient satisfaction metrics.
You will document all encounters accurately and timely in the electronic health record, adhere to regulatory and compliance standards, and participate in quality improvement and care management initiatives driven by AlignMed’s healthcare management model.
The role involves engaging patients and families in shared decision-making, providing clear education about diagnoses, medications, and lifestyle modifications, and facilitating seamless transitions of care between settings and specialties.
You will contribute to clinical protocols, help implement best practices, and support the integration of innovative tools and workflows that improve access, efficiency, and outcomes.
