Job Description
Life Care Centers is seeking a compassionate, dependable LPN Licensed Practical Nurse to join our Healthcare & Medical Services team. In this role, you will provide direct, hands-on nursing care to residents in a senior living and rehabilitation setting, supporting our mission of patient-first, quality care in a family-focused environment. As an LPN with Life Care Centers of America, you will work collaboratively with RNs, therapists, physicians, and other team members to deliver safe, effective, and compassionate care to seniors and adults in need of rehabilitation and long-term support. You will perform routine and complex nursing tasks within your scope of practice, including monitoring and documenting vital signs, administering medications and treatments as ordered, performing wound care, assisting with pain management, and observing residents for changes in condition. You will contribute to the development and updating of individualized care plans, promptly report clinical changes to the supervising nurse or provider, and help coordinate care to support optimal rehabilitation outcomes and quality of life. In this position, you will build meaningful relationships with residents and their families, providing education, emotional support, and clear communication about care plans and daily progress. You will support activities of daily living as needed, promote resident dignity and independence, and help maintain a safe, clean, and respectful environment consistent with our commitment to compassionate, patient-centered care. The LPN will follow all infection control, safety, and regulatory standards, accurately document care and observations in the electronic medical record, and adhere to HIPAA and facility policies. You will participate in interdisciplinary team meetings, contribute to discharge planning and transition of care, and support quality improvement initiatives designed to enhance clinical outcomes and resident satisfaction. This is a team-based clinical role for an LPN who is attentive to detail, comfortable in a fast-paced long-term care and rehabilitation environment, and committed to professional growth. Requirements
Active LPN/LVN license in good standing.
6+ months of clinical experience preferred.
BLS required. IV certification a plus.
Ability to stand for long shifts, lift up to 50 lbs, and assist with patient transfers.
