Responsibilities: GENERAL PURPOSE: In an outpatient setting, remains accountable for provision of direct care to assigned patients.

Contributes to unit activities and supports enhancement of the care delivery system.

This role partners with surgeons, anesthesia, APPs, specialty teams, and external facilities to help support patients through the surgical preparation process.

Responsibilities include coordinating testing and follow-up appointments, communicating with interdisciplinary teams, reinforcing patient education and provider instructions, addressing barriers to care through escalation to the appropriate RN or provider, and supporting cross coverage of the anticoagulation management program.

Candidates must have strong communication, organizational, and critical thinking skills, attention to detail, and a collaborative, patient-centered approach.
