Job Description

Healthcare Nursing of North Bergen, part of Acclaim Rehabilitation, is seeking a compassionate and skilled Nurse Practitioner to join our Healthcare & Medical Services team. In this onsite clinical role, you will provide comprehensive primary and rehabilitative care to residents in our skilled nursing and rehabilitation setting, helping to enhance wellness, support recovery, and optimize quality of life. As a Nurse Practitioner with Healthcare Nursing of North Bergen, you will conduct thorough patient assessments, develop and manage individualized care plans, and collaborate closely with our interdisciplinary team of nurses, physicians, therapists, and social workers. You will evaluate acute and chronic conditions, order and interpret diagnostic tests, prescribe and adjust medications per scope and state regulations, and provide timely interventions to prevent complications and avoid unnecessary hospitalizations. Your work will focus strongly on geriatric and rehabilitative care, including chronic disease management, pain control, wound care, and functional recovery following illness, injury, or surgery. You will play a key role in patient and family education, explaining diagnoses, treatment options, and care goals in a clear and compassionate manner. You will document all care accurately and efficiently in the electronic health record, ensuring compliance with regulatory and facility standards. Collaboration and communication are central to this position; you will participate in care conferences, coordinate transitions of care, and work closely with the nursing and rehabilitation teams to ensure a consistent, patient-centered approach.
