Job Description
Life Care Centers is seeking a compassionate and dedicated RN Registered Nurse to join our Healthcare & Medical Services team at Life Care Centers.
As a key member of a senior living and rehabilitation community, you will provide patient-first, high-quality nursing care to adult and geriatric residents in a family-focused environment.
In this role, you will deliver direct bedside care, perform comprehensive patient assessments, administer medications and treatments, and closely monitor residents’ clinical status to support positive rehabilitation and long-term health outcomes.
You will collaborate daily with an interdisciplinary team—including physicians, therapists, CNAs, social services, and families—to develop, implement, and regularly update individualized care plans focused on safety, comfort, and functional improvement.
The RN Registered Nurse will be responsible for accurate and timely documentation in electronic medical records (EMR), adherence to infection control and fall-prevention protocols, effective pain management, and coordination of transitions of care, including discharge planning and education.
You will provide clear, compassionate communication and education to residents and their families about care plans, medications, and ongoing health needs, while modeling a patient-first, respectful, and supportive approach.

Requirements
Active RN license in good standing.
1+ year of clinical experience preferred; new grads considered for residency tracks.
BLS required. ACLS preferred for hospital roles.
Ability to stand for long shifts, lift up to 50 lbs, and assist with patient transfers.
