How you’ll make an impact in this role Enhanced Communication: RN Patient Navigators serve as a central point of contact, ensuring that patients and families clearly understand complex diagnoses and treatment plans. Care Coordination: They facilitate seamless transitions from the ICU to step-down units or to home, reducing treatment delays and ensuring continuity of care. Barrier Removal: By identifying and addressing obstacles such as financial, transportation, or social issues, they ensure timely access to necessary services. Reduced Readmissions: Providing follow-up support and education on self-management helps decrease emergency room visits and urgent readmissions. Emotional Support: They provide psychosocial support to patients and their families, helping them navigate the distress associated with critical illness. Interdisciplinary Collaboration: Acting as a liaison among doctors, nurses, social workers, and other care team members, RN Patient Navigators optimize care work-flows and foster collaborative practice. What minimum requirements you’ll need Licensure / Certification / Registration: Licensed Registered Nurse obtained prior to hire date or job transfer date required. Registered Nurse credentialed from the Oklahoma Board of Nursing required. BLS Provider preferred. American Heart Association or American Red Cross accepted. Education: Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire.
