Work Shift Day (United States of America) Job Summary: As a Care Partner , you will play a vital role in providing support under the guidance of a Registered Nurse. Your primary focus will be to create a positive patient experience by collaborating with nurses, physicians, and the healthcare team. You'll assist with daily living activities and comfort measures, ensuring that every patient feels cared for and respected. In this dynamic role, you may also engage in advanced technical training, such as phlebotomy and dressing changes, depending on the needs of the unit. Your responsibilities will include: Performing basic patient care functions (ADLs, ambulation, vital signs). Assisting with clerical tasks like data retrieval and maintaining unit supplies. Communicating effectively with the healthcare team and patients. Core Responsibilities and Essential Functions: Excellence in Patient Experience Partner with the healthcare team, patients, and families to: Address patient needs and follow the care plan as directed. Engage patients and families in compliance with treatment plans. Implement safety interventions to minimize risks. Maintain patient confidentiality and adhere to HIPAA regulations. Participate in hourly patient rounding and resolve concerns promptly. Patient Centered Care Provide direct patient care under RN supervision. Assist with hygiene, nutrition, and comfort measures. Support RN and healthcare team members in care delivery. Monitor key patient safety information and participate in charge reconciliation. Teamwork Support healthcare team initiatives and performance improvement. Transport supplies and equipment as needed. Practice accountability in all tasks. Communication Utilize effective communication skills with patients, staff, and families. Document patient care accurately in the EMR. Maintain and communicate updates on patient care equipment. Safety and Infection Control Ensure a safe environment for patient care. Practice proper hand hygiene and infection control measures. Utilize safe handling devices and perform environment rounds. Required Minimum Education: High School Diploma or GED (Preferred) Required Minimum License(s) and Certification(s): BLS - Basic Life Support (required upon hire) ARC-BLS - American Red Cross Basic Life Support (required upon hire) Additional License(s) and Certification(s): Graduation from a State-approved program for Certified Nursing Assistants or Patient Care Tech Program (Preferred) Required Minimum

Experience: Previous direct care experience (preferred) Required Minimum Skills: Ability to follow detailed directions. Strong interpersonal skills. Clear verbal and written communication skills.
