Nurse Practitioner

Working closely with physicians, nurses, and the broader care team, you will conduct comprehensive patient assessments, formulate differential diagnoses, order and interpret appropriate diagnostic tests, and develop individualized treatment and care management plans for a diverse patient population. You will manage both acute and chronic conditions, provide preventive care and wellness counseling, and deliver clear patient and family education to support informed decision-making and long-term health outcomes. As a Nurse Practitioner with AlignMed Partners, you will play a key role in coordinating care across multiple settings and specialties, ensuring continuity, safety, and quality while adhering to evidence-based guidelines and regulatory standards.
