Job Description
AlignMed Partners, a leading healthcare management company based in Addison, TX, is seeking a dedicated Nurse Practitioner to join its Healthcare & Medical Services team. In this role, you will provide high-quality, patient-centered care in a dynamic environment that supports both clinical excellence and professional growth. Working closely with physicians, nurses, and the broader care team, you will conduct comprehensive patient assessments, formulate differential diagnoses, order and interpret appropriate diagnostic tests, and develop individualized treatment and care management plans for a diverse patient population. You will manage both acute and chronic conditions, provide preventive care and wellness counseling, and deliver clear patient and family education to support informed decision-making and long-term health outcomes. As a Nurse Practitioner with AlignMed Partners, you will play a key role in coordinating care across multiple settings and specialties, ensuring continuity, safety, and quality while adhering to evidence-based guidelines and regulatory standards. You will document all encounters and interventions accurately and efficiently within electronic health records, collaborate with interdisciplinary team members to optimize care pathways, and participate in quality improvement initiatives, clinical protocols, and best-practice development.
