Position Number: County: Pulaski Posting End Date: June 2, 2026 Job Duties: Provide Targeted Case Management to children with special health care needs and their families that include screening program applications to determine initial and ongoing medical eligibility; conducting a needs assessment; and developing service plans that bridge clinical care and community resources to improve health outcomes for children with special health care needs.
