Registered Nurse (RN) – Home Health Care - Full Time Pay Range: $43.27 – $58.68 per hour (based on years of experience) Job Summary: Provide skilled nursing care in patients’ homes following physician orders and a comprehensive plan of care.

Serve as the clinical lead during home visits, perform assessments, and coordinate care across the care team.

Qualifications: Current and unrestricted New Jersey RN license.

One to two years of recent acute care or community health experience.

Baccalaureate degree in nursing preferred.

Registered Nurse (RN) – Hospice – Full Time Pay Range: $43.31 - $60.16 per hour (based on years of experience) Job Summary: Provide compassionate, patient-centered hospice nursing care in patients’ homes to ensure comfort and symptom management at end of life.

Serve as a key member of the interdisciplinary care team by assessing patient needs, implementing individualized plans of care, coordinating services, and supporting patients and families emotionally, spiritually, and clinically.

Salary: $89,574 - $139,253 (based on experience) Job Summary: Virtua Health is seeking an experienced Clinical Care Manager to support high-quality home health patient care through clinical oversight, documentation review, staff education, and interdisciplinary collaboration.

This role serves as a resource to clinicians by promoting best practices in care planning, regulatory compliance, quality improvement, and efficient care coordination.

Responsibilities include reviewing and improving clinical documentation and care plans, educating clinicians during orientation and onboarding, conducting recertification reviews, and partnering with operational and IT teams to optimize workflows and Epic care plan functionality.

Qualifications: Current and unrestricted New Jersey RN license.

BSN required; graduate of an accredited School of Nursing.

Minimum of 3–5 years of home care experience required.

Minimum of 2 years of experience in a specialty clinical area.

OASIS coding experience required.

Current BLS certification required.

Collaborate closely with physicians, interdisciplinary team members, patients, and caregivers to coordinate individualized plans of care while promoting quality outcomes and efficient utilization of agency resources.

Responsibilities include conducting comprehensive patient assessments, administering and documenting treatments, educating patients and families, supervising Home Health Aides, and ensuring compliance with documentation and patient safety standards.

Duties include assisting with bathing, dressing, feeding, mobility, light housekeeping, and documentation of care delivered.

Travel within Burlington County is required as part of patient visit assignments.

Qualifications: Current New Jersey Home Health Aide (HHA) certification required.
